How The Test is Done
Simply place one end of 2 cotton swabs in the
mouth under the tongue. Keep the swabs there for a couple
of seconds. Place these samples in a zip-lock bag and seal
it. With a marker, write your name, address and telephone
number on the bag.

How to Order
Simply follow the instructions and return the
Saliva Sample Information Form to us for your results. After
the test has been completed, you will be contacted for a consultation
regarding the results. This may be conducted at our place
of business, by mail or via telephone, whichever is most convenient
for you. Please remember to designate on your saliva sample
bag how you wish to receive your results.
Please complete and return the questionnaire, check (payable to Patti Flora) and swabs in a zip lock bag to us in a Priority Mail Envelope.
